If your family household size and income falls within the guidelines below, you may be eligible for our sliding fee discount. This is available to all patients, whether you have insurance or not.
If you would like to apply, please fill out the application HERE and bring it with you to your next appointment. Proof of income is required and the following forms are accepted: paystubs, tax returns, child support, unemployment benefits, bank statements, W2, social security benefits letter, etc.
If you are claiming no household income, please fill out the Verification of No Income form HERE.
You must re-apply annually for the slide in order to remain eligible.
